Instructions

1. Renew you application each semester and summer. (For summer ONLY contact the instructor).
2. Check all courses (or equivalent) you have either TA'ed before TB, or taken the course (or equivalent) TC.

3. List only those courses for which you are interested in being a teaching assistant for the semester for which you are applying.


4. Do you have any other support (TA / RA / Other) for the upcoming semester? If yes, provide full details below, including the professor, project, and amount of support.
