Work Proposal Form When a student is hired, the student and company representative jointly fill out a proposed work plan for the three semesters of work. Students are eligible to register on the first day of registration after the Work Proposal form is approved by the Co-op director.
Work Experience Survey Students complete this form at the end of each work assignment. The Work Experience Survey is an important tool because it is the primary source of information about what is happening on site. Ratings and comments are summarized and are available at the Co-op office, ME 1120. Students may remain anonymous if they wish; however, they are encouraged to discuss their remarks with their supervisors and to make their responses available to anyone who asks.
Online Request for Reduced Credit For Fall and Spring semester, co-op students must fill out this form by the fourth week of the semester in order to be charged on a per credit basis and still be recognized as a full-time student. Select the “Temporary Change”.
Student Services Fee Assessment Request After filling out the “Online Request for Reduced credit form” you will not be assessed the student services fee. This means you will not have access to the rec center or be provided University Insurance. If you would like access to the rec center and/or to be provided insurance through the University of Minnesota, fill out the Student Services Fee Assessment Request.
Address Update Notify the co-op office if your address changes.
